Case Manager/Clinician

Job Description:

SUMMARY
As part of a multi-disciplinary team, the Clinician will travel to multiple locations to meet and work with clients to attain wellness and recovery and sustain permanent housing. The Clinician will provide intensive mental health and case management services to ensure housing stability, improved health outcomes, increased wellness and recovery, and community integration. The Clinician is responsible for maintaining the highest quality provision of direct individual and group therapeutic services while fostering internal teamwork and continuity of care with other service provider partners.
 
ESSENTIAL DUTIES & RESPONSIBILITIES
  1. While ensuring that medical and intensive case management services to clients emphasize customer choice, harm reduction and psycho-social rehabilitation, maintain a case load of up to 25 clients
  2. Ensure each client that is being case managed has an up-to-date and comprehensive biopsychosocial assessment developed in collaboration with the client and used to create individualized case management plans designed to improve quality of life, health outcomes, and mental health status
  3. Collaborate with each client  to develop their individual service plan; review and update the service plan quarterly and upon completion of goals
  4. Ensure all client  paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases as required by the project and its funding sources
  5. Maintain confidentiality of clients files per agency policies and HIPAA and follow all Policies and Procedures as outlined on HUD  and DHS contracts or any other contracts
  6. In collaboration with Clinical Coordinator/ Clinical Director  and  Case Manager, review all discharge summaries from recently hospitalized clients  to ensure appropriate medical follow-up
  7. Responsible to coordinate appointments, transportation, and follow-up services for clients  accessing primary health care, mental health care, recovery services and need community resources
  8. Travel and transport clients in your vehicle or company vehicle  to various agencies and healthcare providers  to increase community support services and community reintegration
  9. Prepare written reports, and complete related projects as requested
  10. Work cooperatively and cohesively with other members  of the team, including participation in weekly staff meetings, group clinical supervision, and trainings; also, provide clinical consult to other  staff as needed
  11. Develop and maintain a complete, accurate, and current client file with all required documents and data from HUD and VASH; ensure all clients paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases
  12. Adhere to National Association of Social Workers code of ethics

Salary:

$45,000 - $55,000

Job Requirements:

QUALIFICATIONS
  1. Bachelor's degree in related field required for the Case Manager, Master of Social Work required for the Clinician.
  2. Minimum of one year of experience working with homeless individuals, preferably those living with mental illness and or substance addictions, or an equivalent combination of education and experience
  3. Current, valid California Driver’s License with an acceptable driving record and reliable vehicle
  4. Able to work with minimum supervision
  5. Outstanding clinical and strong team-building skills
  6. Detail oriented with excellent time management, organizational, written, verbal, and computer skills
  7. Able to obtain and maintain CPR/1st Aid certification
  8. Minimum 35 WPM typing speed preferred
  9. Bilingual in English/Spanish preferred
